Quick answer: Every ACA Marketplace plan covers pregnancy, childbirth and newborn care as an essential health benefit, with no waiting period, even if you were pregnant before coverage started. Pregnancy alone doesn’t open a Special Enrollment Period, but a birth does (60 days to enroll), and you can apply for Medicaid any time of year. Open Enrollment for 2027 coverage runs November 1, 2026 to January 15, 2027.

How Health Insurance for Pregnancy Works

Health insurance for pregnancy is built into every ACA Marketplace plan. Under the Affordable Care Act, maternity and newborn care is one of the 10 essential health benefits that all ACA plans must cover. This means prenatal care insurance is not an add-on or a rider — it’s included in the base plan at no additional cost.

ACA maternity health insurance covers: prenatal visits, lab work and ultrasounds, gestational diabetes screening, delivery (vaginal and cesarean), hospital stay, postpartum checkups, breastfeeding support and supplies, and newborn care. Many preventive prenatal services, such as recommended screenings, are covered at $0 cost-sharing — no copay, no deductible — on all ACA plans when you use in-network providers.

There is no waiting period for maternity coverage on ACA plans. If you’re already pregnant when you enroll, your pregnancy and childbirth are covered from the day your plan starts; your plan’s deductible, copays and coinsurance still apply. This is unlike some short-term or non-ACA plans that may exclude pre-existing conditions including pregnancy.

Pregnancy as a Qualifying Life Event

Pregnancy alone is not a qualifying life event for Special Enrollment. However, having a baby is. If you’re currently uninsured and pregnant, you have several options: enroll during Open Enrollment (November 1–January 15), apply for Medicaid any time of year (pregnancy income limits are often higher than for other adults), or check whether another event, such as losing other coverage, moving or getting married, gives you a Special Enrollment Period. Plans sold outside the Marketplace are usually a poor fit during pregnancy: the fixed-indemnity plans we’ve reviewed cover complications of pregnancy but don’t list routine maternity care, and other private plans may limit or exclude it. Once the baby is born, you have 60 days to enroll your family in a Marketplace plan, and coverage can start on the date of birth.

What Maternity Coverage Costs on ACA Plans

The cost of pregnancy health insurance depends on your income, household size, and ZIP code. Premium tax credits for 2027 coverage are available when household income is between 100% and 400% of the federal poverty level, and some households pay $0/month after the credit (estimate your 2027 tax credit). If your income is at or below 250% FPL, Silver plans with cost-sharing reductions offer the best value for pregnancy because they have lower deductibles and copays for prenatal visits, labs, and delivery. For 2027, in-network out-of-pocket costs for covered care are capped at $12,000 for one person on any Marketplace plan, and at $4,000 on CSR Silver plans at 100–200% FPL (CMS).

Your Situation

Pregnancy Insurance Options by Situation

Currently Insured Through Employer

Your employer plan covers maternity. After delivery, the baby triggers a qualifying event to add them. FreedInsure can help if employer coverage is too expensive or you want to compare ACA alternatives.

Currently Uninsured & Pregnant

Enroll in ACA during Open Enrollment or apply for Medicaid any time. As of January 2026, the median Medicaid income limit for pregnancy was 201% FPL, ranging from 138% to 324% depending on the state (KFF). Ask us before relying on a private plan during pregnancy. Check eligibility →

Planning a Pregnancy

FAQ

Pregnancy Health Insurance FAQ

Does health insurance cover pregnancy?
Yes — all ACA Marketplace plans cover pregnancy and maternity care as an essential health benefit. This includes prenatal visits, delivery, hospital stay, newborn care, postpartum care, and breastfeeding support. There is no waiting period and no exclusion for pre-existing pregnancy.
Is pregnancy a qualifying life event for health insurance?
Pregnancy alone is not a qualifying life event for ACA Special Enrollment. However, having a baby is. After delivery, you have 60 days to enroll in or change ACA plans. Other qualifying events that may coincide with pregnancy include job loss, marriage, or moving.
What is the best health insurance plan for pregnancy?
A Silver ACA plan with cost-sharing reductions typically offers the best value for pregnancy. If your income is at or below 250% FPL, Silver CSR plans have lower deductibles, copays and out-of-pocket maximums; for 2027, the out-of-pocket maximum is $4,000 for one person at 100–200% FPL, versus $12,000 on a standard plan. A premium tax credit can lower the premium if your income is between 100% and 400% FPL.
How much does pregnancy cost with insurance?
It depends on your plan’s deductible, copays and coinsurance. On any Marketplace plan, your in-network costs for covered care in 2027 can’t exceed the out-of-pocket maximum: $12,000 for one person, or $4,000 on a CSR Silver plan at 100–200% FPL. Many preventive prenatal services are $0 in network. Without insurance, you pay the full bill for prenatal care, delivery and any hospital stay.
Does the ACA cover the baby after birth?
Yes, once you add the baby. A birth opens a Special Enrollment Period: you have 60 days to add the baby to your plan or enroll in a new family plan, and Marketplace coverage can start on the date of birth even if you enroll up to 60 days later. Don’t assume your newborn is covered automatically; rules for the first days after birth vary by plan and state. Every Marketplace plan includes children’s vision coverage, and children’s dental must be available, sometimes as a separate plan with its own premium.
Can I get maternity coverage on a PPO plan?
Yes, if it’s a Marketplace PPO: every ACA Marketplace plan, including PPOs, covers maternity care. Plans sold outside the Marketplace are different: the fixed-indemnity plans we’ve reviewed cover complications of pregnancy but don’t list routine maternity care, and other private plans may cap or exclude it. If you’re pregnant or planning to be, an ACA plan is usually the safer buy. Compare PPO options →
