Quick answer: Supplemental insurance is extra coverage, such as hospital indemnity, critical illness, accident, dental and vision plans, that you add on top of your main health plan to help with costs it leaves behind. Hospital indemnity, critical illness and accident policies pay set cash benefits when a covered event happens. They don’t replace major medical coverage, and on their own they don’t count as minimum essential coverage.

Last updated: October 5, 2026

THIS IS A SUPPLEMENT TO HEALTH INSURANCE AND IS NOT A SUBSTITUTE FOR MAJOR MEDICAL COVERAGE. LACK OF MAJOR MEDICAL COVERAGE (OR OTHER MINIMUM ESSENTIAL COVERAGE) MAY RESULT IN AN ADDITIONAL PAYMENT WITH YOUR TAXES.

Key Details

Understanding Supplemental Insurance

This comprehensive guide covers everything you need to know about supplemental insurance. Whether you’re researching for the first time or looking for specific details, this resource provides expert-level information from licensed insurance professionals.

Who This Guide Is For

Anyone researching supplemental insurance — whether you’re experiencing this situation right now, planning ahead, or helping a family member navigate their options. This guide covers eligibility requirements, available coverage types, enrollment timelines, costs with and without subsidies, and step-by-step instructions for taking action.

Key Concepts

Understanding supplemental insurance requires knowing: whether you qualify for a Special Enrollment Period (60-day window outside Open Enrollment), which plan types are available (ACA, PPO, short-term, supplemental), how subsidies affect your cost, and which carriers serve your area. Each of these factors is covered in detail below.

Your Options

Your Coverage Options

ACA Marketplace Plans

The primary option for most situations. ACA plans from Healthcare.gov cover pre-existing conditions, include all essential benefits, and offer subsidies based on income. Some people still pay $0: in 2026, 29% of HealthCare.gov enrollees picked a plan with a $0 premium after tax credits (CMS). Available during Open Enrollment (Nov 1–Jan 15) or with a qualifying life event (60-day SEP). Check your subsidy →

Private PPO and EPO Plans (Outside the Marketplace)

FreedInsure also offers private PPO and EPO plans sold outside the ACA Marketplace. They use the Cigna PPO network and are not insured by Cigna: the plan, not Cigna, sets benefits and pays claims. They aren’t ACA plans, so you can’t use a premium tax credit on them, and benefits, limits and exclusions vary by plan. You can apply outside Open Enrollment, but approval depends on each plan’s eligibility and underwriting rules, and the start date depends on the plan. Ask us to confirm in writing how a plan handles pre-existing conditions; Marketplace plans must cover them. PPO options →

Important: Plans sold outside the ACA Marketplace may be subject to underwriting, age limits, state availability and each plan’s own eligibility rules. Not all plans are available in all states. Rates are not guaranteed and can change. Benefits, limits and exclusions are set by the official plan document or policy, which controls if it differs from this page. This page is a general summary, not a contract or an offer of coverage. These plans are not sold through the ACA Health Insurance Marketplace (HealthCare.gov or your state’s exchange) and are not individual-market ACA plans. You can’t use a premium tax credit or cost-sharing reduction to pay for them. As HealthCare.gov puts it, “the only way to get the premium tax credit is through the Marketplace.” The Cigna PPO network is a provider network, not an insurance company, and it doesn’t pay claims. A plan that uses the Cigna PPO network sets its own benefits and pays claims through its own administrator. Before you get care, confirm your doctor or facility is in network using the directory and phone number on your plan ID card.

Straight talk: A plan sold outside the Marketplace can cost less than a full-price ACA plan if you’re healthy, earn too much for a tax credit, or want a specific network. But if you qualify for a tax credit or Medicaid, have a pre-existing condition, are pregnant or planning to be, or take expensive medications, an ACA plan is usually the safer buy, and the only way to get the premium tax credit is through the Marketplace. We’ll price both side by side before you decide. Call (844) 788-3733.

Short-Term Medical

Temporary bridge coverage for gaps between plans. $95–$400/month. You can apply outside Open Enrollment, but approval and the start date depend on the plan. How long you can keep it depends on your state and the plan, from 3 months or less in some states to longer terms with renewals where state law allows. Does NOT cover pre-existing conditions. Best for healthy individuals between coverage periods.

Supplemental Coverage

Hospital indemnity ($20–$50/month), critical illness ($25–$50/month), accident ($15–$30/month), dental ($25–$50/month), vision ($10–$20/month). These add benefits on top of a health plan; they don’t replace it. Hospital indemnity is fixed-indemnity coverage: it pays a set dollar amount per covered event, such as a hospital admission or day, no matter what the bill is. It has no out-of-pocket maximum, and you pay any charges above the scheduled amount. Hospital indemnity, critical illness and accident benefits are generally tax-free when you pay the premiums with after-tax dollars (IRS Publication 525); employer-paid or pre-tax premiums can make benefits taxable. Most supplemental policies can be applied for outside Open Enrollment; approval and start dates depend on the policy. Hospital indemnity details →

Costs and Subsidies

ACA With Subsidies

The enhanced subsidies that ran through 2025 expired and are not back for 2027 under current law, so tax credits are smaller than they were. For 2027 coverage, households at 100%–400% of the federal poverty level (about $15,960–$63,840 for one person) are expected to pay about 2.15%–10.22% of income for the benchmark Silver plan (IRS Rev. Proc. 2026-26); above 400%, there is no tax credit. Examples for one person, using the 2026 poverty guidelines that apply to 2027 coverage and assuming no affordable job-based offer: at $23,940 (150% FPL), 4.30% of income is about $86/month; at $31,920 (200% FPL), 6.78% is about $180/month; at $47,880 (300% FPL), 10.22% is about $408/month for the benchmark plan. A cheaper plan, such as Bronze, can cost less, sometimes $0. Estimate yours with the 2027 subsidy calculator →

Without Subsidies

Bronze: $350–$550/month. Silver: $450–$700. Gold: $550–$850. Platinum: $650–$1,000. COBRA: $700–$2,500 (most expensive). Short-term: $95–$400. Private PPO and EPO plans are priced by plan, age, state and eligibility; ask us to quote one next to your ACA options.

Cost-Sharing Reductions

Income at 100–250% FPL? Silver plans get CSR benefits: a lower deductible, lower copays and a lower out-of-pocket maximum. For 2027, the out-of-pocket maximum drops from $12,000 to $4,000 for one person at up to 200% FPL, and to $9,600 at 200%–250% FPL (CMS). This is separate from premium subsidies and only applies to Silver plans bought through the Marketplace. A broker ensures you don’t miss this.

Strategy

How to Take Action

Step 1: Check Your Eligibility

Determine if you have a qualifying life event for Special Enrollment, or if Open Enrollment is active. If neither, Medicaid and CHIP accept applications any time, and you can apply outside Open Enrollment for private PPO/EPO, short-term (where your state allows it) and supplemental plans; approval depends on each plan’s eligibility and underwriting rules. A FreedInsure advisor confirms your eligibility in minutes.

Step 2: Compare Plans With a Licensed Advisor

Call (844) 788-3733 or submit the form below. Your advisor presents 3–5 best options with real after-subsidy pricing from multiple carriers. Free consultation — no obligation.

Step 3: Enroll and Confirm Your Start Date

Pick your plan and enroll over the phone. Total time: 15–30 minutes. All free. Your start date depends on how you enroll: during Open Enrollment, enroll by December 15 for a January 1 start; with most Special Enrollment Periods, Marketplace coverage starts the first of the month after you pick a plan; after a birth or adoption, it can start the day of the event. Plans sold outside the Marketplace set their own start dates after approval. In Washington, DC, we don’t sell ACA Marketplace plans: DC residents enroll through DC Health Link, and we can help with life insurance and supplemental coverage.

Step 4: Add Supplemental (Optional)

After major medical is in place, consider gap protection: hospital indemnity + critical illness + accident + dental/vision. Complete package: $50–$150/month. Accident and critical illness options →

Action Steps

Mistakes to Avoid

Mistake 1: Not Checking Subsidy

KFF estimates that about half of uninsured people were eligible for Medicaid or subsidized Marketplace coverage in 2024, before the enhanced tax credits expired. For 2027 coverage, one person can earn up to $63,840 and still get a tax credit. A 60-second check costs nothing. Check now →

Mistake 2: Choosing by Premium Alone

Cheapest premium ≠ cheapest total cost. Calculate premiums + expected out-of-pocket for true cost.

Mistake 3: Missing Deadlines

60-day SEP windows are firm. Missing = waiting up to 11 months. Start enrollment within the first week of your qualifying event.

Mistake 4: Not Using a Broker

FAQ

Frequently Asked Questions

What are my options for supplemental insurance?
The main types are hospital indemnity, critical illness, accident, dental and vision coverage. They add benefits on top of a major medical plan (ACA, job-based or other) and don’t replace it; on their own they don’t count as minimum essential coverage. A licensed advisor compares options free.
How much does supplemental insurance cost?
It depends on the policy, your age and the benefit amount. Typical ranges we see: hospital indemnity $20–$50/month, critical illness $25–$50, accident $15–$30, dental $25–$50 and vision $10–$20. Hospital indemnity pays a set amount per covered event and has no out-of-pocket maximum, so you pay any charges above the scheduled amount.
Do I qualify for subsidies?
Many people do, for a Marketplace health plan. For 2027 coverage, premium tax credits are available from 100% to 400% of the federal poverty level (about $15,960–$63,840 for one person). The enhanced subsidies that removed the income cap expired after 2025. Tax credits can’t be used for supplemental policies. Check →
When can I enroll?
Supplemental policies can usually be applied for outside Open Enrollment; approval and start dates depend on the policy. ACA health plans: during Open Enrollment (November 1, 2026–January 15, 2027 on HealthCare.gov) or within a Special Enrollment Period after a qualifying life event (usually 60 days). Medicaid and CHIP: any time.
Are pre-existing conditions covered?
Yes on ACA plans. All Marketplace plans must cover pre-existing conditions (HealthCare.gov). Supplemental policies can limit benefits for pre-existing conditions, so check the policy’s terms before you buy. For private PPO, EPO and other plans sold outside the Marketplace, get it in writing before you enroll.
What if I miss Open Enrollment?
Check whether a qualifying life event gives you a Special Enrollment Period, and apply for Medicaid or CHIP any time if you may qualify. Otherwise, private PPO/EPO, short-term (where your state allows it) and supplemental plans can be applied for outside Open Enrollment, but approval depends on each plan’s rules, the start date depends on the plan, and none of them can use a tax credit.
Is FreedInsure free?
Yes. Our help is free to you. For Marketplace plans you get the same plans, prices and tax credits as on HealthCare.gov, plus a licensed advisor’s guidance.
