These dollar figures are derived from the 2026 federal poverty guidelines, which govern 2027 marketplace coverage; the ~138% amounts are rounded. Two more 2027 rules Get Covered Illinois has flagged: starting January 1, 2027, fewer lawfully present immigrants qualify for tax credits (Lawful Permanent Residents, Cuban/Haitian entrants, and COFA migrants keep eligibility), and starting in 2028, customers who don't verify their application information will be renewed without tax credits.

Illinois Medicaid in 2027: Expanded, With New Rules
Illinois has expanded Medicaid under the ACA, so adults earning up to about 138% of the poverty level can qualify — roughly $22,025 for a single adult or $45,540 for a family of four at 2026 guidelines. The program is run by the Illinois Department of Healthcare and Family Services (HFS), and Get Covered Illinois notes it's sometimes called the "medical card." The expansion group, which HFS calls ACA Adults, covers people 19–64 who don't have dependent children under 18 at home and aren't on Medicare. Children are covered through All Kids.
Three changes matter for 2027. (1) Work requirements: HFS says new ACA Adults applicants are subject to the new work requirement if they apply after 5:00 p.m. on December 31, 2026. The federal standard is 80 hours a month of work, school, job training, or community service, or about $580 a month in income, with exemptions for groups such as pregnant and postpartum people, people who are medically frail, and parents of children under 14. (2) Six-month renewals: ACA Adults move to redeterminations every six months starting after their first redetermination date in 2027. (3) Immigrant eligibility: HFS says that as of October 1, 2026, some noncitizens will no longer qualify for federally funded Medicaid.
One catch most people miss: Get Covered Illinois warns that people denied Medicaid for not meeting the work requirement "will not be eligible for tax credits" on the marketplace. The requirement isn't something you can sidestep by switching to a subsidized plan.

Get Covered Illinois Open Enrollment 2027: Dates and Steps
Open enrollment for 2027 coverage begins November 1, 2026, Get Covered Illinois confirmed in August 2026. The end date is the part to watch: as of September 30, 2026, GCI had not published its 2027 deadline and said it will update customers on the new dates in the coming months. Its first open enrollment, for 2026 coverage, was extended twice and finally closed January 31, 2026. Don't assume that happens again.
For comparison, states on HealthCare.gov run November 1, 2026 – January 15, 2027, and enrollees there must sign up by December 15 for coverage starting January 1. Illinois no longer uses HealthCare.gov, so those federal cutoffs don't automatically apply here, but they make a sensible personal target.
