Quick answer: Hawaii expanded Medicaid, so adults earning up to 138% of the poverty level (about $25,337 a year for one person on Hawaii's 2026 guidelines) may qualify for Med-QUEST, the state's Medicaid program. Above that, HealthCare.gov tax credits can bring some plans to $0. Open enrollment for 2027 runs November 1, 2026 to January 15, 2027.

Overview

Health Insurance in Hawaii for 2027: What "Free" Really Means

Free health insurance Hawaii residents can actually get comes from two places. The first is Med-QUEST, Hawaii's Medicaid program, run by the Department of Human Services' Med-QUEST Division. Hawaii has expanded Medicaid, so adults can qualify on income alone. The second is a premium tax credit on Hawaii's health insurance marketplace, HealthCare.gov, which CMS lists as a federally facilitated Marketplace for plan year 2027. If your household income falls between 100% and 400% of the federal poverty level, the credit pays part or all of your monthly premium directly to the carrier, and at lower incomes that math can land on a $0-premium plan. The same rules apply on every island, from Honolulu and the rest of Oahu to Maui, Kauai, Hilo and Kona.

Here's what changed for 2027. The extra "enhanced" tax credits of 2021–2025 expired at the end of 2025, and they are not back for 2027 under current law unless Congress acts. Credits are smaller than they were in 2025, and the 400% FPL cliff is back: earn $1 over the line and your credit is $0. Med-QUEST is also adding a work requirement for many adults in 2027. That makes getting your Hawaii coverage right this year more important, not less.

2027 Income Limits

Hawaii Health Insurance Income Limits for 2027

Hawaii has its own, higher federal poverty guidelines than the mainland. On the 2026 guidelines, 100% of the poverty level in Hawaii is $18,360 for one person and $37,950 for a family of four, versus $15,960 and $33,000 in the 48 contiguous states. Marketplace savings for 2027 coverage are figured on those 2026 guidelines. The IRS sets how much of your income you're expected to pay for the benchmark Silver plan, from 2.15% to 10.22%, and the tax credit covers the rest.

2027 Household IncomeHousehold of 1 (about)Household of 4 (about)What You Typically Get
Up to 138% FPLUp to $25,337Up to $52,371Likely Med-QUEST for adults 19–64 (2026 guideline figures — see below)
138–150% FPL$25,337–$27,540$52,371–$56,925The biggest credits — you pay no more than 4.30% of income for the benchmark Silver plan, and 94% CSR Silver plans are available
150–200% FPL$27,540–$36,720$56,925–$75,900You pay 4.30%–6.78% of income for benchmark Silver, plus 87% CSR Silver plans that shrink deductibles
200–250% FPL$36,720–$45,900$75,900–$94,875You pay 6.78%–8.66% of income; 73% CSR Silver plans still trim cost sharing
250–400% FPL$45,900–$73,440$94,875–$151,800You pay 8.66%–10.22% of income — credits taper but still cut real dollars off the premium
Over 400% FPL$73,440+$151,800+No premium tax credit for 2027 under current law — full-price plans, so shopping smart matters most

Dollar figures are simple math on Hawaii's 2026 poverty guidelines, so treat them as approximate. Med-QUEST uses the current year's guidelines, so its dollar limits will update once HHS publishes the 2027 guidelines, which it normally does each January; marketplace credits keep using the 2026 guidelines for all of 2027. Children qualify for Med-QUEST at much higher incomes (see below), so in many families the kids are on Med-QUEST while the parents use a marketplace plan.

Income means your projected 2027 household income — W-2 wages, 1099 and gig income, self-employment, everything. Getting it right matters more than ever: for tax years after 2025 there is no cap on repaying excess advance credits, so a low estimate can mean paying back the full difference at tax time. Hawaii Medicaid

Med-QUEST in 2027: Who Qualifies and What's Changing

Hawaii expanded Medicaid, and the program is run by the Department of Human Services' Med-QUEST Division, which calls its members QUEST members. Med-QUEST lists these income limits for people whose eligibility is based mainly on income:

GroupMed-QUEST Income Limit
Adults under 65 (not on SSI or Medicare)Up to 138% FPL (about $25,337 for one person on the 2026 guidelines)
Parents and caretaker relativesUp to 105% FPL
ChildrenUp to 313% FPL
Pregnant womenUp to 196% FPL

Med-QUEST doesn't use an open enrollment window, so you can apply any time. Apply online through KOLEA, Med-QUEST's eligibility application, at medical.mybenefits.hawaii.gov, or call Med-QUEST at 1-800-316-8005 (TTY 711). Free interpreters are available.

New for 2027: Med-QUEST work requirements

Med-QUEST says new requirements take effect January 1, 2027 for most adults ages 19–64 who are not blind, disabled or pregnant. Hawaii checks them at renewal: they don't apply until your 2027 renewal, and you need to meet one of the three tests below (income, school, or work and volunteering) in at least one of the six months before it.

Income

Household income of $580 or more in a month. Seasonal workers may use their average monthly income over six months.

School

Enrolled at least half time in an educational program.

Work or volunteering

At least 80 hours in a month, which can combine a job or internship, job training such as Job Corps, less-than-half-time school, and volunteering with an organization that tracks hours.

Renewals every six months

For this adult group, Med-QUEST renewals move to every six months instead of once a year, so keep your address and phone number current.

Med-QUEST names exempt groups including children (0–18), adults 65 and older, and people who meet SNAP or TANF work requirements, among others, so check your status with Med-QUEST before you assume anything. Under federal law, people denied Medicaid for not meeting the requirement can't get marketplace tax credits instead.

Immigrant households: two dates to know

Federal law narrowed Medicaid eligibility for some immigrants on October 1, 2026. Then on January 1, 2027, premium tax credits narrow too: only lawful permanent residents, Cuban/Haitian entrants and COFA migrants (from the Marshall Islands, Micronesia and Palau) stay eligible, while refugees, asylees, parolees and TPS holders lose them. For Hawaii's COFA communities, that means marketplace credits stay available in 2027 for those who otherwise qualify. If your household includes immigrants, call before you assume a plan's price — the math can change completely.

Straight talk: Med-QUEST is a state program, not something we sell. If it's your best option, we'll tell you exactly that and point you to KOLEA — no charge, no strings. And if your income sits above the Med-QUEST line, that's precisely where marketplace credits take over, and where a side-by-side comparison of every plan available to you matters most.

Income near the line? Households with seasonal work, gig income or variable hours can move between Med-QUEST and the marketplace from year to year. In most cases, losing Medicaid eligibility opens a 60-day Special Enrollment Period for marketplace coverage. Don't guess at the boundary; a 10-minute call maps it.

Job-Based Coverage

Hawaii's Prepaid Health Care Act: Check Your Job First

Hawaii has its own employer coverage law. Under the Prepaid Health Care Act, originally enacted in 1974, employers must provide health coverage to employees who work at least 20 hours a week and earn at least 86.67 times the current Hawaii minimum wage a month, according to the state Department of Labor and Industrial Relations (DLIR).

When coverage starts

After four consecutive weeks of employment, or the earliest time after that the plan allows.

What you can be charged

Your share can't exceed the lesser of 50% of the premium or 1.5% of your monthly gross earnings.

When you can waive it

DLIR lists cases such as being covered by Medicare or Medicaid, as a dependent under a qualified plan, or under your own personal policy.

Why it matters for credits

If your job offers affordable coverage, you generally can't get marketplace tax credits. For 2027, it counts as affordable if your share of the lowest-cost self-only plan is no more than 10.22% of household income.

So if you're self-employed, between jobs, or working fewer than 20 hours a week, the marketplace or Med-QUEST is usually where your coverage comes from. Working for yourself? See our self-employed insurance guide.

Your Options

Marketplace Insurance in Hawaii: Carriers & Plans for 2027

Hawaii's marketplace runs on HealthCare.gov, and every plan sold there is ACA-compliant: the 10 essential health benefits, no pre-existing condition exclusions, and preventive care at no cost. Carriers and plans can change from year to year, so the only lineup that matters is the one for your ZIP code. We confirm which carriers actually offer 2027 plans where you live, and check them against your doctors and prescriptions, before recommending anything. (See our carrier partners.)

Bronze

Lowest premiums — they can be $0 after credits at lower incomes — with the highest deductibles. Exchange bronze plans now count as HSA-eligible, and the 2027 HSA limit is $4,500 for self-only coverage.

Silver

The sweet spot for most subsidy-eligible households. Under ~250% FPL, Cost-Sharing Reductions can cut a 2027 out-of-pocket maximum from $12,000 to as low as $4,000 for one person.

Gold

Higher premium, lower out-of-pocket. The smart pick when you know care is coming — a planned procedure, a pregnancy, ongoing prescriptions.

Add-Ons

Dental & vision, critical illness, and hospital indemnity plans stack alongside your ACA plan, and term life insurance protects the people who depend on you.

One honest warning: a $0 premium is not $0 care. Free bronze plans carry real deductibles, and a 2027 marketplace plan can have an out-of-pocket maximum as high as $12,000 for one person. If your income is under ~250% FPL, a CSR Silver plan is often the far better deal than the "free" one. That's the judgment call a broker exists for.

2027 Open Enrollment

Hawaii Open Enrollment 2027: Key Dates

Hawaii uses the federal Marketplace, so the HealthCare.gov calendar applies: Open Enrollment runs November 1, 2026 – January 15, 2027. Hawaii doesn't run its own exchange for 2027. You can compare every state on our 2027 open enrollment deadlines by state page.

November 1, 2026

Open enrollment opens on HealthCare.gov. Compare 2027 plans, premiums, and your new credit amount.

December 15, 2026

Last day to enroll for coverage that starts January 1, 2027. This is the deadline that matters most.

January 15, 2027

Open enrollment ends. Enroll between December 16 and January 15 and coverage starts February 1, 2027 — a one-month gap if you have no other coverage in January.

After January 15

You'll need a qualifying life event (losing job coverage, losing Medicaid, moving, marriage, or a birth) to open a 60-day Special Enrollment Period. Med-QUEST takes applications any time.

Enrolling through FreedInsure takes about 15 minutes: (1) we project your 2027 income and calculate the exact credit, (2) we compare the plans available where you live against your doctors and prescriptions, and (3) we submit the enrollment and stay on as your advisor all year. Same plans, same prices as going direct — plus a human who answers. Call (844) 788-3733. For the national picture, read our Open Enrollment 2027 guide.

Straight talk on auto-renewal: if you do nothing, most enrollees are automatically re-enrolled in the same or a similar plan. But with credits smaller and premiums changing, last year's plan at last year's assumptions can be this year's overpay. Never let a plan auto-renew without a 15-minute re-shop — and do it before December 15 so any change starts January 1.

FAQ

Free Health Insurance Hawaii FAQ: 2027 Answers

Is free health insurance real in Hawaii for 2027?
Yes, through Med-QUEST and premium tax credits. Adults 19 to 64 earning up to 138% of the poverty level (about $25,337 for one person on Hawaii's 2026 guidelines) may qualify for Med-QUEST, Hawaii's Medicaid program. Above that, some lower-income households can find $0-premium plans on HealthCare.gov after their credit. Credits are smaller than in 2025 under current law, so compare before assuming last year's price still applies.
What income qualifies for Med-QUEST in Hawaii?
Med-QUEST lists limits of 138% of the poverty level for adults under 65 (not on SSI or Medicare), 105% for parents and caretaker relatives, 313% for children and 196% for pregnant women. On Hawaii's 2026 guidelines, 138% is about $25,337 a year for one person and $52,371 for a family of four. Those dollar figures will update when the 2027 guidelines take effect.
What income qualifies for subsidized health insurance in Hawaii in 2027?
For 2027 coverage, premium tax credits apply between 100% and 400% of the poverty level. Hawaii uses its own, higher guidelines, so that's about $18,360 to $73,440 for a single person, or about $37,950 to $151,800 for a family of four. Over 400% FPL, there's no premium tax credit for 2027 under current law.
When is open enrollment in Hawaii for 2027?
November 1, 2026 to January 15, 2027, on HealthCare.gov. Enroll by December 15 for coverage starting January 1; enroll December 16 to January 15 and coverage starts February 1. Hawaii doesn't run its own exchange for 2027. After January 15, you need a qualifying life event for a 60-day Special Enrollment Period, but you can apply for Med-QUEST any time.
What are the new Med-QUEST work requirements in 2027?
Starting January 1, 2027, most adults 19 to 64 who aren't blind, disabled or pregnant must meet one of these in at least one of the six months before their Med-QUEST renewal: household income of $580 or more in a month, at least half-time school, or 80 hours of work, job training or volunteering in a month. The rules don't apply until your 2027 renewal, and renewals for this group move to every six months.
Do employers in Hawaii have to offer health insurance?
Generally, yes. Hawaii's Prepaid Health Care Act requires employers to cover employees who work at least 20 hours a week and earn at least 86.67 times the Hawaii minimum wage a month, starting after four consecutive weeks of work. Your share can't exceed the lesser of 50% of the premium or 1.5% of your monthly gross earnings.
What insurance companies offer marketplace plans in Hawaii?
Check your ZIP code. Carrier lineups can change every year, so a list from a past year can mislead you. You can compare the 2027 plans for your ZIP code on HealthCare.gov once open enrollment opens November 1, 2026, or we can check your options, and your doctors, for you. Call (844) 788-3733.
Does it cost anything to use a health insurance broker in Hawaii?
No, our help is 100% free to you. Same plans, same prices as enrolling on HealthCare.gov yourself, plus an advisor who handles enrollment and problems all year. Call (844) 788-3733.
